Please contact our office for referral forms at 805-967-5010. Dr. Carley is able to meet with referring Dr.'s to review cases in a team approach. Please schedule a phone or in person meeting to discuss how Dr. Carley can help your patients achieve optimal chewing and oral health.
Dr. Carley has recently acquired a 3D CT scanner. For patients that will be treated in Dr. Carley's office please schedule an Implant Exam or general Surgical consult first and then the CT scan will be taken as needed.
